	We would like to reserve one 10x10 space (We understand we will supply tent, weights for tent, table and chairs - no refunds regardless of weather)
	I understand set-up is between 8-10 AM and that all vehicles must be off premises by 10:30 AM sharp! I have read this, and agree to a timely set up onsite the day of event. *
	I confirm that I have vendors insurance and/or a policy that covers my participation in this event (Please bring a copy of this to event) *
	In consideration of acceptance, I the applicant agree to indemnify, hold harmless, and to protect and defend Mamapalooza Inc (including the subsidiaries, officers, directors, employees, counsel) from any and all claims by any person or entity for loss or damage to property and for personal injuries including death, which may arise out of the negligence or willful misconduct of myself, employees, agents and contractors directly or indirectly in connection with my booth, performance, presentations *
